9am-9pm; rotating 12 hour shifts (Example of schedule- Week 1 working days: Monday, Tuesday, Friday, and Saturday Week 2 working days: Sunday, Wednesday and Thursday)

Responsibilities

Primary Job Functions:

  1. Clinical Responsibilities:
  2. Demonstrates the ability to assess the consumer’s needs Demonstrates the ability to assess the consumer’s needs utilizing the SUN-R
  3. Identifies goals and plans for appropriate services documented in ISP
  4. Provides appropriate linkage to community resources
  5. Monitors consumers to secure placement in the least restrictive environment
  6. Provides ongoing monitoring of consumers’ progress and needs as assessed in the SUN-R
  7. Advocates on behalf of the consumer to secure community resources
  8. Effectively collaborates with the treatment team members and outside sources in the delivery of
  9. Works closely with treatment team and ensures that the team is aware of any
  10. Assists in de-escalating crisis situations with consumers and/or their families
  11. Effectively demonstrates the application of adult specific competencies
  12. Provides outreach services to consumers and families
  13. Provides training of basic living skills to improve consumers quality of life
  14. Assists consumers with transportation needs to psychiatric appointments
  15. Makes multiple contact per week with consumers, depending on consumer needs
  16. Promptly engages consumers in services following referral
  17. Monitors for compliance with outpatient commitment orders and reports and non-compliance
  18. Clinical responsibilities a. Assesses consumers’ financial resources to determine the need to apply for benefits.
  19. Assists consumers in completing applications to secure benefits.
  20. Assists consumers in obtaining documentation required for applications to Social Security Administration.
  21. Advocates on behalf of consumers with the Social Security Administration office.
  22. Effectively utilizes the SOAR system.
  23. Tracks consumers’ statuses that have applied for benefits.
  24. Ensures the Finance and Accounting department and treatment team is notified when benefits are obtained.
  25. Ensures matching of appropriate services for multiple consumers in case load.
  26. Seeks clinical supervision and consultation as needed
  27. Participates in weekly staff meetings to address concerns regarding consumer with supervisor and other members of the treatment team
  28. Accepts and employs suggestions for improvement
  29. Actively works to enhance case management skills
  30. Documents in a timely manner per AltaPointe standards, within 24 hours of provision of service
  31. Documents appropriate, complete, and comprehensive SUN-R assessments
  32. Documents measurable and specific goals in the individualized service plan
  33. Documents in a clear and concise manner, consumer problems, ISP goals addressed and services provided
  34. Includes problem, ISP/Treatment plan goal, intervention provided, consumer responses, and plans for continued services in each progress note
  35. Documents case disposition, case closings, and follow-up
  36. Documents legibly.  
  37. Effectively manages caseload based on consumers’ needs.
  38. Meets AltaPointe productivity standards.  
  39. Courteous and respectful towards consumers, visitors and co-workers a. Treats consumers with care, dignity and compassion.
  40. Respects consumers’ privacy and confidentiality.
  41. Is pleasant and cooperative with others.
  42. Assists consumers and visitors as needed.
  43. Personal values don’t inhibit ability to relate and care for others.
  44. Is sensitive to the consumers’ needs, expectations and individual differences.
  45. Is gentle and calm to consumers and families.
  46. a. Actively participates in Performance Improvement activities.
  47. Actively participates in AltaPointe committees as required.
  48. Completes assigned tasks in a timely manner.
  49. Follows AltaPointe policies and procedures.
  50. Attends required in-service trainings and other workshops.

Qualifications

Bachelor’s degree in a mental health discipline; certification of case management training by Alabama State Department of Mental Health and Mental Retardation; ability to stay organized and multi-task a workload with multiple consumers;

  • knowledge in adult specialty, outlined below, to be attained within six months of employment:
  • Knowledge of psychiatric disorders in the adult population
  • Knowledge of cultural diversity
  • Knowledge of behavior management, crisis intervention and interventions common to acute psychotic consumers, as well as, to non-violent crisis intervention practice
  • Knowledge of psychiatric rehabilitation principles
  • Knowledge of available community resources
  • Knowledge of educational methods appropriate to adults with severe and persistent psychiatric disorders
  • Knowledge of the social security system in order to obtain benefits
  • Knowledge of the SOAR system
