Shift/Hours
8:00 AM - 8:00 PM; rotating schedule including every other weekend (Example of schedule- Week 1 working days: Monday, Tuesday, Friday, and Saturday Week 2 working days: Sunday, Wednesday and Thursday)

Responsibilities

Clinical Responsibilities:

 

  1. Responds to crisis calls in a timely manner
  2. Makes face to face visits when calls come into Access to Care or the Crisis Response Intensive Care Coordinator
  3. Provides services in the community, consumer’s homes, etc.
  4. Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
  5. Effectively determines the level of response needed in crisis situations
  6. Assists in contacting family members if needed
  7. Communicates effectively with Access to Care to assist in finding appropriate placement when necessary
  8. Evaluates the consumer for community referrals needed
  9. Educates families on de-escalating crisis situations, including but not limited to, safety plans
  10. Assists in securing emergency medical services or law enforcement if needed
  11. Advocates on behalf of the consumer to secure community resources
  12. Effectively collaborates with the treatment team members and outside sources in the delivery of services
  13. Follows up with consumers that are in AltaPointe Children’s Outpatient Programs who may be decompensating or at high-risk
  14. Travels to consumer’s homes to ensure medication compliance as needed
  15. Utilizes appropriate communication methods to ensure appropriate communication with individual consumers’ treatment teams
  16. Manages and triages calls from multiple referral sources
  17. Effectively demonstrates the application of adult specific competencies
  18. Provides transportation to services on a limited basis

 

Supervision and Consultation:

 

  1. Seeks clinical supervision and consultation as needed
  2. Accepts and employs suggestions for improvement
  3. Actively works to enhance case management skills

 

Clinical Record Keeping:

 

  1. Documents in a timely fashion per AltaPointe policy
  2. Documents using Avatar appropriately
  3. Documents appropriate, complete, and comprehensive assessments
  4. Documents in detail the resolution of crisis situations and linkage to appropriate follow up sources
  5. Documents in a clear and concise manner services provided before the completion of the shift
  6. Documents legibly

 

Case Load Management:

 

  1. Effectively manages caseload based on consumer needs
  2. Meets AltaPointe productivity standards if applicable

Qualifications

Bachelor’s degree in a mental health discipline, or a nurse, or a certified parent peer specialist with lived experience working with crisis situations.

Must be at least age 23, maintain a valid verifiable driver license and maintain a clear driving record.

 

Knowledge in child/adolescent specialty, outlined below, to be attained within (6) six months of employment:

  • Knowledge of psychiatric disorders in the child/adolescent population
  • Knowledge of cultural diversity
  • Knowledge of behavior management, crisis intervention and interventions common to acute psychotic consumers, as well as, to non-violent crisis intervention practice
  • Knowledge of Psychiatric Rehabilitation principles
  • Knowledge of available community resources
  • Knowledge of educational methods appropriate to children/adolescents with psychiatric disorders
  • Knowledge of substance abuse
  • Knowledge of AltaPointe Health Systems programs and services
