Responsibilities

Primary Job Functions:

 

Clinical Responsibilities:

  • Provides effective clinical assessment of served populations
  • Formulates evidence-based clinical treatment plan in collaboration with patient
  • Utilizes assessment data to support a DSM-5 diagnosis
  • Provides individual, group, and family therapy, utilizing clinical evident therapeutic models and theories according to patient needs as stated in the treatment plan
  • Effectively collaborates with the Family Medicine treatment team in effort to provide integrated healthcare
  • Provides patient centered safety planning and safety resources appropriate for populations
  • Evaluates patient’s treatment for discharge planning
  • Assures that program standards and objectives are met
  • Plans and develops creative and relevant therapeutic interventions according to patient’s abilities and needs
  • Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
  • Effectively collaborates with community resources to ensure wholistic care and assess for needed referral needs of patient
  • Tracks clinical outcomes
  • Effectively demonstrates the application of adult and child/adolescent specific competencies
  • Adheres to professional code of ethics

 Supervision and Consultation:

  • Seeks clinical supervision and consultation as needed
  • Accepts and employs suggestions for improvement
  • Actively works to enhance clinical skills

 Clinical Record Keeping:

  • Documents in a timely fashion per AltaPointe/Accordia Health policy
  • Documents appropriate, complete, and clinically comprehensive mental health assessments
  • Documents measurable and specific goals in the treatment plan
  • Documents in a clear and concise manner, patient problems, treatment goals addressed, services, and techniques used
  • Documents case disposition, case closings, follow-up and referrals
  • Documents legibly and clinically

 Case Load Management:

  • Effectively manages caseload for quality, standards, and identifying patient needs
  • Meets AltaPointe/Accordia Health clinical productivity standards

Courteous and respectful attitudes towards consumers, visitors and co-workers

  • Treats consumers with care, dignity and compassion
  • Respects consumer’s privacy and confidentiality
  • Is pleasant and cooperative with others
  • Assists consumers and visitors as needed
  • Personal values don’t inhibit ability to relate and care for others
  • Is sensitive to the consumer’s needs, expectations and individual differences

 Administrative and Other Related Duties as Assigned:

  • Actively participates in Performance Improvement activities
  • Actively participates in AltaPointe/Accordia Health committees as required
  • Completes assigned tasks in a timely manner
  • Accesses appropriate community resources according to patients needs
  • Follows AltaPointe/Accordia Health policies and procedures
  • Maintains current license and requirements for renewals (i.e. CEU’s), and attends required in-services and workshops

Qualifications

Master’s degree in a mental health discipline. Licensure preferred, or eligible for licensure process for specific discipline.  Knowledge in adult, child and adolescent specialties as outlined below:

  • Knowledge of psychiatric disorders in child, adolescent and adult populations
  • Basic knowledge of substance-related and addictive disorders
  • Knowledge of integrated-care practices within primary-care setting
  • Basic knowledge of senior adult mental health symptomology and disorders
  • Knowledge of behavior management, crisis intervention, and evidence-based interventions
  • Knowledge of community resources as applied to the child/adolescent and adult populations
  • Knowledge of educational methods appropriate to adult and child/adolescent populations
  • Knowledge of growth and development as related to all intellectual, emotional, spiritual, and physical aspects of the patient’s development
  • Knowledge of developmental and intellectual disabilities child/adolescent population
  • Knowledge of family dynamic and parenting skills
  • Knowledge of effects and treatment of physical/sexual/emotional abuse and neglect
  • Knowledge of cultural diversity
  • Knowledge of private insurance for mental health services
  • Knowledge of FQHD, CCBHC, and Joint Commission standards
