The FACT Case Manager position is situated within the Behavioral Health Crisis Unit. The Forensic Assertive Community Treatment (FACT) team provides mobile community based mental health treatment to promote the recovery and community integration of individuals with serious mental illnesses, high rates of co-occurring substance use disorders and histories of criminal justice involvement. The FACT team serves consumers on parole, probation, returning to the community from jails and prisons, participating in alternative-to-incarceration and treatment court services and those with histories of high-risk behavior. The Forensic Assertive Community Treatment Case Manager works in collaboration with the FACT team to deliver effective treatment and rehabilitation services to reduce episodes of hospitalizations, excessive use of ERs, medical and psychiatric inpatient beds, arrests, and incarceration for program consumers.
Primary Job Functions:
Bachelor’s degree in a mental health discipline and certification of case management training by Alabama State Department of Mental Health and Mental Retardation.
