General Responsibilities:
- Prepares charts for clinic appointments with Providers per guidelines: Identify Care Guidelines required, Check for quarterly labs due, Check for GPRA’s due initial and every 6 months
- Provide information gathered in Chart Prep to team members in Huddle the day of clinic visit
- Initiate the clinic visit by greeting all patients in a professional and timely manner – within 10 minutes of sign-in with office professionals
- Assist with the completion of the following: height/weight, waist measurement, vital signs (report any abnormal results to Provider prior to Provider seeing patient)ask to determine if any Care Guidelines have been met since last visit (ex. Mammogram completed, etc, if so, task referral specialist to get results) Complete Referral for any Care Guidelines that have Standing Orders that have not been
- Complete documentation in NextGen to include the following sections: allergies, vitals, past medical surgeries, social section to include tobacco (provide tobacco cessation counseling for identified smokers), caffeine, alcohol, military, animals, blood to blood, religious preference, PHQ2/PHQ9, UNCOPE and referral to Substance Abuse treatment if positive, Update patient medication section in NextGen, Put a check mark in medications reconciled box, Complete 1 pg. Review of Systems/enter details to elaborate if negative, HIV Hep C Screen, Move patient to waiting for Provider and assist in arranging hospital admissions by calling in report if necessary
- Assist Provider during examinations when needed
- Order injections from pharmacy 5-7 days before due to administer and ensure all injections are received prior to injection date
- Prepare and administer injections including by intramuscular, intradermal, and subcutaneous injections- including vaccinations/immunizations, observing the Seven Rights of medication administration as directed by Provider and documents in NextGen, ensure patient is scheduled for return injection
- Follow up for any missed injections to get them rescheduled and document accordingly
- Complete in-house labs and provide laboratory results to the Provider – alerts Provider of any abnormal results immediately
- Assist in collecting and preparing laboratory specimens that need to be sent to laboratory
- Take electrocardiograms
- Wound care and dressings
- Notify nurse manager when laboratory or medical supplies needed
- Check nurse line for phone message and respond to patient phone calls within 24 hours
- Secure verbal order for prescription refills from Provider and send electronically via NextGen and documents in NextGen
- Facilitate PA’s for prescriptions
- May assist instructing patients about medication and special diets
- Provide emergency care as needed (CPR, MindSet,etc.), according to AltaPointe Policy & Procedures
- Demonstrate the ability to recognize the elements of a crisis state and how to deescalate or resolve the situation
- Effectively demonstrates the application of CMA competencies
- Ensure all releases are obtained and scanned into point of service timely and accurately.
- Verify all insurance eligibility and enters accurate information in EHR as indicated
- Make copies, fax documents and performs other related clerical duties as needed.
- Adheres to professional code of ethics
Additional job duties depending on location may be as follows:
- Ensures controls are run on all CLIA waived tests per manufacturers guidelines
- Assist nurse with completing monthly sample and stock medication logs by the 5th of the following month and sends to Performance Improvement
- Assist with the patient assistance pharmacy forms as needed
- Complete monthly refrigerator log and send to ADPH
- Perform relief for other staff members.

- Seek clinical supervision and consultation needs

Clinical Record Keeping:
- Document in a timely fashion per AltaPointe policy
- Documentation shows that forms, notes, logs, and other applicable written information are completed promptly, accurately and updated when appropriate

- Maintain current State certifcation requirements for renewals (i.e. CEU’s), and attends to required in-services and workshops

Physical Requirements of the Job:
- Must be able to stand for long periods of time, continuously bend, stoop, kneel, crouch, reach and lift at least 10 pounds. Must have visual acuity in at least one eye and manual dexterity.
- There is frequent standing while getting files, paperwork, and handing out medication.
- Sitting is frequent by nature and up to 5-10 minutes; however, extended sitting could be up to 1.5 to 3 hours for documentation.
- Squatting and/or lunging frequently to stock supplies and retrieve needed material from file cabinets and book shelves.
- Fine manipulation for writing, handling paperwork, and dispensing medications to consumers.
- Pushing and/or pulling force up to 1-5 pounds force, horizontally, to open file cabinets and place binders on book shelves.
- Lifting various ranges from the floor to overhead with weights up to 1-5 pounds frequently.
- Any other duties as assigned by supervisor
