Responsibilities

Primary Job Functions:

Clinical Responsibilities

  • Demonstrates the ability to assess recipient needs and strengths utilizing the CANS
  • Identifies individualized goals and objectives and plans for appropriate services
  • Monitors recipients to secure placement in the least restrictive environment
  • Provides ongoing monitoring of recipients’ progress and needs as assessed in the CANS
  • Advocates on behalf of the recipients to secure community resources
  • Effectively collaborates with the NOVA Mobile First Episode Psychosis Program  treatment team members and outside sources in the delivery of case management services to ensure goals are being achieved
  • Ensures that services provided are meaningful, respectful and promote self-determination
  • Effectively demonstrates the application of youth/young adult specific competencies
  • Participates in the development of crisis intervention and support plans for each participant
  • Assists with meeting the needs of participants by facilitating their ability to live independently, obtain employment, achieve educational goals, and/or function in the community more effectively
  • Provides transportation or links to transportation services when necessary
  • Provides psycho-educational services and co-leads multi-family groups
  • Performs other duties as a NOVA Mobile First Episode Psychosis Program team member as assigned by the supervisor

 

Supervision and Consultation

  • Seeks clinical supervision and consultation as needed
  • Accepts and employs suggestions for improvement
  • Actively works to enhance case management skills

 

Clinical Record Keeping

  • Documents in a timely fashion per AltaPointe policy
  • Documents appropriate, complete, and comprehensive CANS assessments to identify  recipient needs, strengths, and planned interventions
  • Documents measurable and specific goals in the individualized service plan
  • Documents in a clear and concise manner ISP goals addressed with each service provided
  • Maintains current documentation of participant demographic information, releases of information, etc.
  • Documents case disposition, case closings, follow-up and referrals

 

Courteous and respectful attitudes towards recipients, visitors and co-workers

  • Treats recipients with care, dignity and compassion
  • Respects recipient’s privacy and confidentiality
  • Is pleasant and cooperative with others
  • Assists recipients and visitors as needed
  • Personal values don’t inhibit ability to relate and care for others
  • Is sensitive to the recipient’s needs, expectations and individual differences
  • Is gentle and calm to recipients and families

 

Case Load Management:

  • Effectively manages caseload based on recipient needs
  • Meets AltaPointe productivity standards

 

Administrative and Other Related Duties as Assigned:

  • Actively participates in Performance Improvement activities
  • Actively participates in AltaPointe committees as required
  • Completes assigned tasks in a timely manner
  • Follows AltaPointe policies and procedures
  • Attends to required in-service trainings and other workshops 

Qualifications

 

Bachelor’s degree in a mental health discipline.  Knowledge in youth/young adult specialty, outlined below, to be attained within six (6) months of employment:

  • Knowledge of psychiatric disorders in the youth/young adult population
  • Basic knowledge of growth and development of youth/young adult recipients as related to all intellectual, emotional, spiritual, and physical aspects of their development
  • Knowledge of developmental disabilities and mental retardation in the youth/young adult population
  • Basic knowledge of behavior management and parenting issues
  • Knowledge of cultural diversity
  • Knowledge of behavior management, crisis intervention and interventions common to acute psychotic recipients, as well as, to non-violent crisis intervention practice
  • Knowledge of effects of physical/sexual/emotional abuse and neglect issues
  • Knowledge of available community resources as applied to youth/young adult
  • Certified by the Alabama State Department of Mental Health to provide adult and/or youth/young adult case management services 
  • Knowledge of the CANS (Child and Adolescent Needs and Strengths Assessment)
  • Experience facilitating behavioral health therapy groups with children, adolescents, and adults
  • Valid Alabama’s driver’s license
