Responsibilities

Primary Job Functions:

Adult Foster Homes

  • Monitors basic living skills billing for accurateness and timely submission
  • Provides basic living skills teaching/instructions and monitoring, based upon needs identified in DLA-20
  • Completes monthly monitoring of foster homes for adherence to standards and contract and follows up on noted deficiencies
  • Monitors move in and out of the program and ensures census accuracy
  • Processes medical consults and incident reports and enters incident reports in RL-6 Incident Reporting program
  • Reviews monthly vouchers with Finance & Accounting department
  • Keeps up to date records of DLA-20s on each foster home patient

 Supported Employment

  • Monitors and maintains program referral tracking forms
  • Prepares monthly reports for Alabama Department of Mental Health (ADMH) and Vocational Rehabilitation (VR)
  • Monitors Avatar documentation, including Direct Service Record and program specific forms, e.g., Career Profile, Job & Education Start/End, Job & Education Search, Job & Education Support, Disclosure, Enrollment and Employer Contact Log forms
  • Monitors Vocational Rehabilitation’s Milestone report due dates and required supporting documentation

 PATH, Shelter + Care (S+C) and Permanent Housing (PH)

  • Monitors and maintains program referral tracking forms
  • Monitors and updates HMIS, per HUD and CoC requirements
  • Monitors and maintains inventory for PATH outreach kits and S+C and PH move-in kits

 ARBOR COURT

  • Ensure routine monitoring of homes is completed for adherence to environmental standards and program rules
  • Submits work orders when needed
  • Monitors move in and out of the program and census accuracy
  • Processes incident reports and enters incident reports in RL-6 Incident Reporting program

Record Keeping

  • Documents in a timely manner per AltaPointe policy
  • Documents in a clear and concise manner
  • Documents legibly
  • Learns and utilizes AvatarNx, RL-6, HMIS, Premiere, WorxHub, and Excel
  • Monitors and maintains Adult Community Services (ACS) vacancy tracker form
  • Monitors and maintains ADMH unreserved vacancy report

 General Responsibilities:

  • Greets patients
  • Answers telephone
  • Records telephone messages accurately and transfers to appropriate person in a timely manner
  • Maintains office inventories and orders supplies
  • Orders supplies for ACS programs to ensure all are equipped per program standards
  • Makes copies, faxes documents and performs other related clerical duties as needed
  • Protects and maintains the confidentiality of all records and patient information
  • Handles routine requests from department staff
  • Verifies insurance for patients
  • Other duties as assigned by Supervisor
  • Provides transportation for patients

  • Attends appropriate in-services and other workshops

Qualifications

Minimum Qualifications: 

High School diploma or equivalent. Must be 23 years of age or older; must have and maintain reliable transportation, a valid driver license, acceptable driving record, and proof of current liability insurance. Skilled in Microsoft Office tools: Word, Excel, PowerPoint, OneNote and Outlook.

Knowledge in adult specialties, outlined below, to be attained within (6) six months of employment.

  • Knowledge of psychiatric disorders in the adult population
  • Knowledge of cultural diversity
  • Knowledge of behavior management, crisis intervention and interventions common to acute psychotic patients, as well as, to non-violent crisis intervention practices.
  • Knowledge of psychiatric rehabilitation principles
  • Knowledge of available community resources
  • Knowledge of educational methods appropriate to adults with severe and persistent psychiatric disorders
  • Knowledge of basic living skills
