Overview

The Coordinator, Adult Community Services position supervises a variety of community-based programs, including both single apartments and congregate housing with case management support, and homeless outreach. These service options help AltaPointe’s adult, outpatient consumers move forward in their recovery, access local resources to meet their unique needs, achieve a better quality of life and function as independently as possible in their community.

Responsibilities

Supervises Adult Community Services program operations: (Arbor Court, VOA, Supportive Housing, Permanent Housing, Shelter + Care, and PATH):

  • Directs and organizes program activities
  • Establishes program goals and objectives that support the organization’s strategic plan
  • Identifies problems, evaluates alternatives, and implements effective solutions
  • Maintains and monitors electronic tracking of referrals
  • Manages wait lists
  • Evaluates referrals for eligibility, coordinating and staffing with members of candidate’s treatment team(s)
  • Staffs all referrals with supervisor, prior to program entry
  • Coordinates with Adult Community Services’ Property Manager regarding moves in and out of housing, furniture replacements, remediation of bug infestations, and evictions
  • Coordinates Permanent Housing and Shelter + Care referrals from Coordinated Entry (CCN) and documents referral outcomes in Homeless Management Information System (HMIS)
  • Ensures Permanent Housing, Shelter + Care, and PATH staff are properly trained and licensed in HMIS, and that HMIS is kept up to date
  • Monitors PATH documentation in electronic heath record and HMIS, and ensures PATH enrollment is being utilized effectively
  • Monitors staff productivity and documentation, ensuring staff are providing program participants with quality and timely services
  • Completes general and crisis notes in electronic health record
  • Monitors and assists staff with money management, case management, basic living skills, and homeless outreach activities
  • Provides guidance and strategies to staff when program participants are out of compliance, or when program participants are preparing for discharge
  • Meets with program participants to address program expectations and concerns
  • Accesses appropriate community resources for program participants

 

Administers appropriate personnel functions:

  • Interviews and screens job applicants when necessary, and completes appropriate forms as outlined by HR Policies and Procedures
  • Provides training to employees in the general orientation of the facility, policies and procedures, program operations, and specific skill areas
  • Objectively completes employee performance evaluations on supervised staff and routes to HR on a timely basis
  • Promotes concept of treatment team(s) and serves as a member of treatment team(s)
  • Provides positive and effective leadership
  • Provides staff with consultation and supervision as needed, individually or through group staff meetings
  • Conducts weekly unit staffings with all direct report staff
  • Participates in staff development training to provide staff with best practices

  • Accepts and employs suggestions for improvement
  • Actively works to enhance leadership skills
  • Completes assigned tasks in a timely manner

Qualifications

Master’s degree in a mental health discipline with at least two (2) years post master’s degree experience. Knowledge, outlined below, to be attained within six (6) months of employment.

  • Knowledge of all housing programs provided by Adult Community Services
  • Knowledge of homeless services provided by Adult Community Services
  • Knowledge of policies and procedures and eligibility criteria for programs provided by Adult Community Services
  • Knowledge of grants and contracts concerning housing and homeless services provided by Adult Community Services
  • Knowledge of psychiatric disorders in the adult population
  • Knowledge of co-occurring psychiatric and substance use disorders
  • Knowledge of behavior management and interventions, including crisis intervention practices, for adults with both chronic and acute symptoms of serious mental illness
  • Knowledge of psychiatric rehabilitation principles
  • Knowledge of educational and motivational principles appropriate to adults with psychiatric and co-occurring disorders
