Responsibilities

Primary Job Functions:       

 

Clinical responsibilities

  • Completion of medical intake forms for patients including: Medical Checklist/Physical Health/Nutritional screening, Medication List, Medication Administration Record, vital signs, and admission notes.
  • Prepare and administer injections including intramuscular, intradermal, and subcutaneous injections observing the Seven Rights of medication administration as directed by Provider and documents in Avatar. This is done both in the office and in the patient’s home.
  • Order injections from pharmacy 5-7 days before due to administer and ensure all injections are received prior to injection date
  • Organizes and tracks when patient injections are due, the site where they were last administered, the frequency, and the pharmacy information
  • Follow up on any missed injections to get them rescheduled and document accordingly
  • Provides medication education for the patient and family, including uses and potential side effects.
  • Monitors patients for potential side effects of medications and reports findings to the physician. – This is done both in the office and in the patient’s home.
  • Monitors patient medication compliance and assists in ensuring medications are secured and taken appropriately
  • Communicates effectively with the physician regarding the patient’s physical and mental condition
  • Interfaces with the multi-disciplinary team and actively and cooperatively participates in development of the interdisciplinary treatment plan
  • Performs venipuncture for laboratory requests, monitor results, and reports all abnormal findings to the physician
  • Provides emergency care as needed (CPR, MindSet, PRN medications), according to AltaPointe Policy & Procedures
  • Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
  • Effectively demonstrates the application of adult competencies
  • Adheres to professional code of ethics

Supervision and consultation

  •  Seeks clinical supervision and consultation needs.
  • Accepts and employs suggestions for improvement
  • Actively works to enhance clinical skills

Clinical record keeping

 

  • Documents in a timely fashion per AltaPointe policy.
  • Documents in a clear, concise manner, types of patient problems, treatment goals and services provided
  • Documentation shows that forms, notes, logs, and other applicable written information are completed promptly, accurately and updated when appropriate
  • Documents legibly

 Courteous and respectful attitudes towards patients, visitors and co-workers

 

  • Treats patients with care, dignity and compassion

Qualifications

Possesses current certification for a Certified Clinical Medical Assistant. Must be able to stand for long periods of time, continuously bend, stoop, kneel, crouch, reach and lift at least 10 pounds. Must have visual acuity in at least one eye and manual dexterity. Must have the ability to communicate verbally, withstand continual deadlines, concentrate and maintain accuracy in spite of frequent interruptions.

 

Knowledge of adult specialties, outlined below, to be attained within six (6) months of employment:

  • Knowledge of psychiatric disorders in the adult populations
  • Knowledge of general and psychiatric nursing care
  • Knowledge of growth and development of individuals as related to all intellectual, emotional, spiritual, and physical aspects of their development
  • Knowledge of substance use disorders and treatment
  • Knowledge of intellectual and developmental disabilities
  • Basic knowledge of medical terminology in psychiatric care
  • Familiarity of psycho-pharmacy and the use of psychotropic medications with the adult populations
  • Demonstration of safe medication administration practices including IM injection
  • Knowledge of behavior management, crisis intervention, and interventions common to acute psychotic patients as well as to non-violent crisis intervention practice
  • Knowledge of instruction, teaching tools, educational methods appropriate for adult populations
  • Knowledge of family dynamics
  • Knowledge of effects and treatment of physical/sexual/emotional abuse and neglect issues
  • Knowledge of cultural diversity
  • Knowledge of ADMH and CCBHC standards
