Adult Outpatient Programs provide the least restrictive level of care where adults may receive clinical interventions aimed at decreasing existing signs and symptoms. Outpatient treatment focuses on recovery, problem resolution, symptom reduction, stabilization and/or prevention of movement to a higher level of care. Treatment varies widely and depends upon the consumer’s presenting symptoms.


Primary Job Functions:

Clinical Responsibilities:

  • Develops and maintains activity/class calendar and assigns staff appropriately, conducts staffing’s as needed.
  • Responsible for daily operations of the Intensive Day Treatment program
  • Adheres to all standards relative to the program
  • Assesses, screens, and reviews appropriateness of all consumers enrolled in the program.
  • Completes Extension of Stay notes on consumers every 90 days as indicated.
  • Maintains accurate enrollment of all consumers in the program
  • Reviews and reconciles all weekly day treatment weekly notes in a timely and accurate manner and submits notes for approval no later than Monday of the following week
  • Provides at least 1 hour of therapy for each consumer enrolled in Intensive Day Treatment weekly; if not provided, appropriate documentation is included in the weekly note to explain.
  • Documents therapy notes concurrently and according to standards
  • Provides effective clinical assessment of the consumer
  • Utilizes assessment data to support a DSM V diagnosis
  • Formulates appropriate treatment plan along with consumer
  • Provides individual, group, and family therapy, utilizing basic therapeutic models and theories according to consumer needs as stated in the treatment plan
  • Completes intakes for consumers coming into outpatient treatment
  • Completes aftercares for consumer returning for treatment after hospitalization
  • Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
  • Effectively collaborates with the treatment team members and outside sources in the delivery of clinical services
  • Evaluates the consumer for discharge planning
  • Effectively demonstrates the application of adult specific competencies
  • Adheres to professional code of ethics


Supervision and Consultation:

  • Seeks clinical supervision and consultation as needed
  • Accepts and employs suggestions for improvement
  • Actively works to enhance clinical skills


Clinical Record Keeping:

  • Documents in a timely fashion per AltaPointe policy
  • Documents appropriate, complete, and clinically comprehensive mental health assessments
  • Documents measurable and specific goals in the treatment plan
  • Documents in a clear and concise manner, types of consumer problems, treatment goals addressed, services, and techniques used
  • Documents case disposition, case closings, follow-up and referrals
  • Documents legibly


Case Load Management:

  • Effectively manages caseload based on consumer needs
  • Meets productivity standards


Courteous and respectful attitudes towards consumers, visitors and co-workers

  • Treats consumers with care, dignity and compassion
  • Respects consumers privacy and confidentiality
  • Is pleasant and cooperative with others
  • Assists visitors and consumers as needed
  • Personal values don’t inhibit ability to relate and care for others
  • Is sensitive to the consumers needs, expectations and individual differences

Administrative and Other Related Duties as Assigned:

  • Actively participates in committees as required
  • Completes assigned tasks in a timely manner
  • Accesses appropriate community resources according to consumer needs
  • Follows AltaPointe Health Systems policies and procedures
  • Maintains current license and requirements for renewals (i.e. CEU’s), if applicable, and attends to required in-services and workshops


Master’s degree in a mental health discipline.  LISCW Required
