Responsibilities

  1. Effectively collaborates and consults with staff members, including clinicians and pharmacists, and outside sources in the delivery and arrangement of care management services
  2. Assists in consumer discharge planning and appointment scheduling
  3. Accurately populate fields to enroll consumers in electronic health records and care management software
  4. Review patients on clinical schedules and access electronic medical record to identify chronic conditions
  5. Transcribe clinical data into care management platforms
  6. Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
  7. Provides support and education for family members of consumers
  8. Meets AltaPointe productivity standards

Qualifications

Bachelor degree in a business, health information technology, healthcare, or social services related discipline required. Seeking a candidate with excellent verbal and written communication skills, highly organized, and self-motivated. Experience working in the healthcare field preferred. Skills, outlined below, are preferred and/or must be attained within the first six months of employment:

  • Knowledge of electronic health records and care management software
  • Knowledge of standards for the Alabama Department of Human Resources and Department of Mental Health
  • Knowledge of Alabama DHR, Medicare, Medicaid, and other insurers’ billing requirements
  • Knowledge of social determinants of health
  • Knowledge of available referral sources for physical health, mental health and social services
  • Knowledge of psychiatric disorders in child and adult populations
  • Knowledge of chronic medical conditions
  • Knowledge of cultural diversity
