  • Overview

    Adult Outpatient Programs provide the least restrictive level of care where adults may receive clinical interventions aimed at decreasing existing signs and symptoms. Outpatient treatment focuses on recovery, problem resolution, symptom reduction, stabilization and/or prevention of movement to a higher level of care. Treatment varies widely and depends upon the consumer’s presenting symptoms.

    Responsibilities

    • Demonstrates the ability to assess the individual’s wants and needs through an interviewing process.
    • Identifies the wants and needs and plans for appropriate services and interventions.
    • Provides appropriate linkage to community resources
    • Monitors consumer to secure placement in the least restrictive environment
    • Monitors individuals in each service area
    • Provides ongoing monitoring of individuals’ progress of their  wants and  needs as assessed in the Individual and Family Profile
    • Advocates on behalf of the individual to secure community resources
    • Develop the Case Management Plan
    • Assist the individual and family in obtaining Medicaid eligibility
    • Face-to-face contact required a minimum of every 90-days
    • Development of the 90-day narrative
    • Assist in providing community integration
    • Implement Person-Centered holistic manner
    • Ensure the health and safety of the individual in all areas
    • Monitor attendance in all Medicaid funded services
    • Effectively collaborates with  team members and outside sources in the delivery of case management services
    • Evaluates the individual for discharge planning
    • Monitors the MR/DD Waiver Plan of Care

     

    Qualifications

    Bachelor’s degree in a mental health or mental retardation discipline

    Maintain a valid Alabama driver license and must have proof of current liability insurance

    Must be 23 years of age or older
