  • Overview

    Case Managers assist with monitoring consumer’s well-being including needs in the areas of mental health, physical health, living, learning, working, and social environments. Case managers travel to wherever consumers reside, whether that is at home, boarding homes, or other community based living to provide monitoring, when working with children they would be going to the schools as well.  The primary role of the case manager is to increase psychological adjustment for the consumer by empowering the consumers and sometimes family members with information, education and support in order to follow through once case management ends.

     

    Responsibilities

Case managers travel to wherever consumers reside, whether that is at home, boarding homes, or other community based living to provide monitoring, when working with children they would be going to the schools as well.  The primary role of the case manager is to increase psychological adjustment for the consumer by empowering the consumers and sometimes family members with information, education and support in order to follow through once case management ends.

     

    Essential Functions

    • Responds to crisis calls in a timely manner
    • Makes face to face visits when calls come into Access to Care
    • Provides services in the community, consumer's homes, etc
    • Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
    • Effectively determine the level of response needed in crisis situations
    • Assists in contacting family members if needed
    • Communicates effectively with Access to Care to assist in finding appropriate placement when necessary
    • Evaluates the consumer for community referrals needed
    • Educates families on de-escalating crisis situations including but not limited to safety plans
    • Assists in securing emergency medical services or law enforcement if needed
    • Effectively collaborates with the treatment team members and outside sources in the delivery of services
    • Follows up with ocnsumers that are in AltaPointe's Adult Outpatient programs who may be decompensating or at high risk
    • Travels to consumer's homes to ensure medication compliance as needed
    • Utilizes appropriate communication methods to ensure appropriate communication with individual consumers' treatment teams
    • Handles and triages calls from multiple referral sources
    • Effectively demonstrates the application of adult specific competencies
    • Provides transportation to services on a limited basis
    • Provides case management for consumers
    • Provides a Human Service Needs Assessment at required frequency
    • Develops an Individual Service Plan for each case management consumer
    • Works with the Crisis Team Leader to decrease admissions to psychiatric hospitals using the least restrictive alternative necessary to address a consumer's recovery
    • Follows through with services determined by the Crisis Team Leader to divert consumers from commitments to psychiatric facilities
    • Seeks clinical supervision and consultation as needed
    • Accepts and employs suggestions for improvement
    • Actively works to enhance case management skills
    • Documents in a timely fashion per AltaPointe policy
    • Documents using Avatar appropriately
    • Documents appropriate, complete, and comprehensive assessments
    • Documents in detail the resolution of crisis situatons and linkage to appropriate follow up sources
    • Documents in a clear and concise manner services provided
    • Documents legibily
    • Maintains mileage reimbursement logs efficiently and with integrity
    • Effectively manages case load based on consumer needs
    • Meets AltaPointe's productivity standards
    • Treats consumers with care, dignity and compassion
    • Respects consumer's privacy and confidentiality
    • Is pleasant and cooperative with others
    • Personal values don't inhibit ability to relate and care for others
    • Is sensitive to the consumer's needs, expectations and individual differences
    • Is gentle and calm with consumers and families

    • Actively participates in AltaPointe committees as required
    • Completes assigned tasks in a timely manner
    • Follows AltaPointe policies and procedures
    • Attends required in-service training and other workshops

    Physical Requirements

    • Frequent standing, sitting, and walking which are in bursts of 5 to 10 minutes.
    • There are times when this can be extended to pacing with the consumer (time could be undetermined) and at times sitting and standing can be extended with the employee having the ability to change positions as permitted by the observance and behavior of the consumer at that time.
    • Occasional forward bending from the hips while kneeling to reach to the side.
    • Occasional need to be able to assume a full squat/lunge from a standing position.
    • Occasional need to maintain back extension while being on stomach with one’s own body weight on hips and legs. This position is maintained for several minutes.
    • Occasionally, be able to put one’s own body weight on one knee and one elbow on the same side of one’s body.
    • Occasionally, be able to kneel and reach to same side toward the floor.
    • Lift up to 10 pounds occasionally during shift.
    • Strong grasp is needed to use keys to open doors throughout the facility, and to maintain constant contact with a consumer during a take down procedure or with other hold techniques.
    • Pushing/pulling force up to 25 pounds occasionally during a shift.

    Qualifications

    Bachelor's degree in a mental health discipline and experience working with psychiatric disorders or crisis situations

    Completes the DMH case management training after hire

    Has and maintains a valid driver's license

    Must be 23 years old or older

    Has proof of liability insurance
