  • Overview

    CarePointe is AltaPointe's access-to-care department and the single point of entry into the majority of AltaPointe's programs and services. CarePointe is responsible for coordinating consumer care throughout our continuum, which may include facilitating hospital admissions and outpatient appointments, verifying insurance coverage, making discharge arrangements following hospitalization and providing referrals and linkage to other community resources. Through a brief telephone screening, trained staff members assess an individual’s needs for care, triage any crisis that the caller may be going through, and determine the most appropriate level of care and service. The team members' have behavioral healthcare experience and training. Their educational credentials include bachelor and graduate degrees in mental health disciplines or related fields.  

    Responsibilities

    1. Provides clinical and administrative training, consultation, and supervision to Access staff
    2. Coordinates and supervises day to day activities and documentation
    3.  Facilitates intake assessments and admission to AltaPointe program services ensuring appropriateness of level of care through use of clinical criteria ensures continuity of care, including aftercare services, of consumers during and after stay in psychiatric hospitals or other facilities.
    4. Oversees UM/UR activities, including written and statistical reports, special projects, and quarterly committee meetings
    5. Ensures staff productivity through regular review of EMR and computerized call center/ distribution software reports
    6. Monitors psychiatric consultation service contracts and oversees coordination of those services with local hospitals.
    7. Collaborates regularly with Center and BayPointe hospital staff to facilitate admissions, referrals, scheduling, and overall coordination care.
    8. Identifies problems, evaluates alternatives and implements solutions to improve procedures that support programmatic goals and the organization’s strategic plan.
    9. Educates and communicates regularly with community providers and referral sources about accessing AHS services and maximizing continuity of care.
    10. Works closely with the Information Technology department to secure appropriate consumer and benchmarking data.
    11. Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
    12. Effectively demonstrates the application of specific competencies listed above
    13. Adheres to professional code of ethics

    CarePointe Specialists, Financial Investigation Planner, Crisis and Hospital Coordination Specialist, Hospital Admission and Referral Coordinator, Relief, Crisis and Hospital Intake Specialist, Switchboard Operators, and Aftercare and Referral Specialist

    Qualifications

    Master degree in a mental health discipline with licensure (LPC, LICSW).  At least five years post graduate clinical experience required.
