  • Overview

    The Assisted Outpatient Treatment Team serves individuals that find it difficult to come to an AltaPointe facility for desperately needed treatment. These patients are court ordered to attend treatment through the probate court. They usually have been hospitalized multiple times over the course of their illness, have a limited or no support system, inadequate basic living skills and persistent difficulty in illness and medication management. Other complicating factors may involve substance abuse problems and legal issues or other stresses that significantly impair their ability to maintain mental stability. These patients need intensive support from our AOT Team. The AOT team is a mobile, multi-disciplinary group of professionals who deliver the treatment and services needed in the manner and location that is most comfortable for the patient. The Team’s mission is to improve the patient’s situation so that they can transition to a less intensive type of treatment. Outpatient treatment focuses on recovery, problem resolution, symptom reduction, stabilization and/or prevention of movement to a higher level of care. Treatment varies widely and depends upon the consumer’s presenting symptoms. Nurses are responsible for educating patients about their medication and the benefits of taking it consistently, encouraging medication and treatment compliance, assisting patients with understanding the relationship between mental illness, their medications, and substance use, assisting patients with gaining access to appropriate healthcare for physical needs and providing continuity of care between mental and physical healthcare providers.


    1. Completes the assessment process accurately and timely for consumers including medical checklist, physical health, nutritional screening, consumer and family education screening, medication list, height, weight, and vital signs.
    2. Responds to consumers’ requests for medication refills.
    3. Reviews charts for treatment compliance.
    4. Completes prior authorization forms in a timely manner.
    5. Provides individual, family and group education regarding consumers’ psychiatric and medical illnesses.
    6. Administers medications observing the five rights of medication administration.
    7. Provides medication education for the consumer and family including how the medication is used and potential side effects.
    8. Monitors consumers for potential side effects of medications and reports findings to a physician.
    9. Provides care and services in the community.
    10. Transports needed medical equipment/supplies to conduct job.
    11. Performs venipuncture for laboratory requests, drug screens, TB skin tests, monitors lab results and reports all abnormal findings to the physician.
    12. Monitors consumers for potential side effects of medications and reports findings to the physician/CRNP.
    13. Communicates effectively with the physician/CRNP regarding the consumers’ physical and mental condition
    14. Follows up on missed injections and documents in consumers’ records appropriately.
    15. Completes admission procedures for consumers and updates accordingly to AltaPointe’s Policy and Procedures. This process includes at a minimum the medical checklist, AIMS, vital signs, medication list, and MARS is applicable.
    16. Interacts with consumers and evaluates needs to ensure that proper information is reported during staffing with the treatment team.
    17. May conduct infection control training for staff and consumers when indicated.
    18. May provide HIV education.
    19. When necessary provides nursing coverage to other adult outpatient programs within AltaPointe.
    20. Interfaces with the multi-disciplinary team.
    21. Actively and cooperatively participates in development of the interdisciplinary treatment plan.
    22. Independently provides mental and medical emergency care as needed (CPR, PMAB, emergency medications) according to AltaPointe Policy and Procedures with little/no support.
    23. Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation

    25. Assists AOT consumers with medical appointments including attendance at those appointments as needed.
    26. Provides services to maintain placement in the least restrictive environment.
    27. Advocates on behalf of the consumer to secure community resources.
    28. Makes a minimum of one weekly in home contact with consumers.
    29. Teaches basic living skills relative to medical issues.
    30. Adheres to professional code of ethics.
    31. Effectively demonstrates the application of adult specific competencies.


    Current RN license to practice in the State of Alabama; valid driver license; reliable transportation; knowledge in adult psychiatric care and/or substance abuse/addictions, outlined below, to be attained within six (6) months of employment:

    • Knowledge of psychiatric disorders in the adult population
    • Knowledge of general and psychiatric nursing care
    • Basic knowledge of medical terminology especially those terms associated with adult psychiatry care
    • Familiarity of psychopharmacology and the use of psychotropic medications with the adult population
    • Knowledge of cultural diversity
    • Knowledge of behavior management, crisis intervention and interventions common to acute psychotic consumers, as well as, to non-violent crisis intervention practice
    • Knowledge of AOT team standards
    • Basic knowledge of available community resources
    • Knowledge of educational methods appropriate to adults with severe and persistent psychiatric disorders
    • Knowledge of educational methods appropriate to adults with psychiatric disorders
    • Knowledge of psychiatric rehabilitation principles
    • Understanding of behavioral, psychological, physical health and social effects of psychoactive drugs, including alcohol and tobacco on the consumer and their significant others
    • Basic teaching and training skills
