  • Responsibilities

    Primary Job Functions:

    • Provides ongoing monitoring and care coordination for consumers on caseload
    • Completes enrollment of all consumers into the Care Management software and follows up with assigned tasks in a timely manner
    • Completes administrative forms in order to open charts
    • Completes both written and verbal releases
    • Assists in having consumers complete PHQ-9 and       Alcohol Assessment, obtains vital signs, and list of current medications prior to psychiatric evaluations, and physicians appointments as indicated
    • Follows up on all missed appointments
    • Provides support on behalf of the consumers in order to secure community resources and needed services
    • Ensures primary medical treatment team is informed of AltaPointe services being provided and updates them accordingly on the status of services
    • Effectively collaborates and consults with staff members, including clinicians and pharmacists, and outside sources in the delivery and arrangement of care management services
    • Assists in meeting with consumers referred by Franklin Medical staff to assess their needs and assist in getting them set up for services with AltaPointe as appropriate
    • Provides Franklin Medical staff with documentation of services provided for their medical charts
    • Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
    • Applies CPR, Mindset and restraint interventions per AltaPointe Policy and Procedures
    • Provides support and education for consumers and the family members of consumers
    • Delivers or arranges clinically necessary transportation
    • Provide documentation of services rendered in the appropriate space indicated to include but not limited to consumer problem(s), interventions provided, and consumer response to interventions, referrals, collaboration with other care managers, follow up appointments, and community outreach services, etc.
    • Discharges consumers in avatar
    • Assists consumers with obtaining benefits when applicable
    • Other duties assigned by supervisor
    • May provide outreach services to consumers and families on occasion

    Other Job Duties:

    • Greets all consumers in a professional and welcoming manner.
    • Schedules, cancels and re-schedules appointments as necessary.
    • Ensures all consumers sign in for their appointments
    • Answers telephone and directs calls appropriately
    • Receives consumer requests for appointments/information and changes.
    • Records all telephone messages accurately and transfers to appropriate person timely
    • Handles routine requests from clinicians.
    • Keys in no shows daily.
    • Directs consumers to appropriate waiting area
    • Handles unscheduled consumers with professionalism and empathy.
    • Promptly notifies appropriate clinician of consumer’s arrival.
    • Schedules return appointments for consumer accurately.
    • Reviews DSR for appointment information
    • Gives consumer appointment card with next scheduled appointment and initials.
    • Protects and maintains the confidentiality of all records and patient information.
    • May perform quality assurance process as necessary.
    • Completes administrative review audits as necessary.
    • Makes copies, faxes documents and performs other related clerical duties as needed.
    • Ensure all releases are obtained and scanned into P.O.S. timely and accurately.
    • Accurately populates all fields as related to the admission process in the Avatar system as related to the patient’s insurance and guarantor rankings.
    • Researches all patients that present with no insurance through Medicaid and Medicare eligibility sites.
    • Verifies all insurance eligibility and enters accurate information into Avatar.
    • Accurately collects “out-of-pocket” costs (i.e. co-pays, deductibles and co-insurance).
    • Balances out cash box/drawer accurately (daily) and prepares deposit.
    • Adheres to all 5 Star Customer Service guidelines when dealing with Practice Management, Team Leader, co-workers and patients.
    • Passing out prescriptions from the doctor/CRNP/PA to the consumer.
    • Assists with the patient assistance pharmacy forms as needed, which includes communicating with the consumer regarding proof of income.
    • Completes admissions, discharges, and transfers as needed.
    • Performs relief for other staff members.
    • Assists with and works with all organization personnel involved with any aspect of release of protected health information to ensure full coordination and cooperation under the organization’s policies and procedures and legal requirements

    Physical Requirements of the Job:

    • There is frequent standing while getting files, paperwork, and handing out medication.
    • Sitting is frequent by nature and up to 5-10 minutes; however, extended sitting could be up to 1.5 to 3 hours for documentation.
    • Squatting and/or lunging frequently to stock supplies and retrieve needed material from file cabinets and book shelves.
    • Fine manipulation for writing, handling paperwork, and dispensing medications to consumers.
    • Pushing and/or pulling force up to 1-5 pounds force, horizontally, to open file cabinets and place binders on book shelves.
    • Lifting various ranges from the floor to overhead with weights up to 1-5 pounds frequently.
    • Any other duties as assigned by supervisor

    Qualifications

    Bachelor’s degree in a healthcare related discipline. Active CMA, CNA, or MA in the state of Alabama required.

    Must be 23 years of age or older, maintain a valid Alabama driver license and have proof of current liability insurance.

    Must have the ability to communicate verbally, withstand continual deadlines, concentrate and maintain accuracy in spite of frequent interruptions.
